Faculty Coursebook Requisitions

To submit requisitions for the Professional Bookstore, use the Professional Bookstore form. 

1. Submit only one course at a time.
2. Bulleted items are required.
3. List titles in the order they will be used during the semester.
4. If you have over five (5) titles you will have to submit this form once for each set of five titles.
5. If no text is required, please check appropriate box and send.
6. You are responsible for notifying your Department Chairperson or Program Coordinator if necessary.
7. The order confirmation will be sent to the Instructor's NYU E-mail address entered below.

   Please include the publisher's name for each book request. Failure to do so may delay your request.
